The Full Outline of Unresponsiveness (FOUR) score is a coma rating scale proposed for critically ill patients. Unlike the Glasgow Coma Scale, it does not depend on verbal responses, which makes it usable when speech cannot be assessed, such as in intubated or aphasic patients.
